Someone posted on Gamefaqs asking what game this is from:
and so far no one has been able to place it. The "NOPE" was probably added afterwards (and is not part of the game), of course, but what is the game called?

A member of Vogons thinks it is Scorpion: Disfigured (no, I've never heard of it, either)
